A decision is only reviewable if you can see what the decider knew at the time. Each entry records the decider, the information they used, the divisions affected, and the date. It is the same record the morning briefing cites.

Entries are append-only. A reversal is a new entry that points at the one it replaces, so the history stays readable.

Why the information column exists

A decision made on partial information is not a mistake by default. It becomes one when nobody can tell, later, which part was missing.
It also gives the council something to check. If a decision cites evidence that never reached the org memory graph, that gap is itself a signal.
Reversals stay visible. The entry that supersedes an older one is linked to it, rather than replacing it quietly.
Owners become comparable. Two divisions deciding the same question with different evidence is a pattern worth seeing before it costs money.
